Do you accept health insurance?
In order to serve clients without restrictions from insurance companies, I am an out-of-network practice with all insurance companies. Clients pay me directly each visit for their 1-on-1, individualized care. However, you may still be able to be reimbursed by your insurance benefits. Please request documentation at your visit if you would like to submit your claim to your insurance company for out-of-network reimbursement.
I have chosen to be an out-of-network provider to emphasize quality care and to focus on your goals, without being limited by what your insurance company will allow us to work on.
How do I find out my out-of-network benefits?
Call your insurance company and ask:
What are my out-of-network benefits for outpatient physical therapy?
Do I have an out-of-network deductible?
What paperwork do I need to submit for direct reimbursement?
What address should I mail this paperwork to, or is there an online submission option?
What are the prices for your services?
Initial evaluations and follow up sessions will be $125 for 60 minutes. Payment can be made via cash or check.
Do I need a physician referral?
No, Massachusetts has unrestricted, direct access for physical therapy. This means that you will never need a referral in order to be seen.
Do I have to be a member of Pioneer Valley CrossFit to receive your services?
No, I am located inside of Pioneer Valley CrossFit, however clients do not have to be gym members in order to participate in physical therapy, wellness, and performance services. The location does mean we can make use of gym equipment, as needed, to progress towards your goals.
Is there Parking Available?
Yes, there is free on-site parking available at the gym.
